Direct Support Associate(s) or Home Care Aide(s) :
Provides support
for individuals and/or groups of persons with disabilities both in residential and day settings. Assists the person(s) receiving
services in identifying and communicating his or her requests and needs for services and supports. Implements services and
supports to address the service user’s goals and desired outcomes as identified through each service user’s Person
Centered Plan (PCP)
Assists service
users to identify personal outcomes that best suit their personal interests, cultural background, and desires for the future.
Assists the Team in identifying supports needed for personal outcomes to be realized. Understands and promotes the service
user’s goals, as well as personal dignity, respect, individualism and quality of life
Provides opportunities
for choice, and encourages service users to make choices, and to exercise control over themselves and their environment
Supports service
users in eating, resting, dressing, bathing, grooming, toileting, playing, and working, according to each person’s abilities
and interests. Helps service users develop healthy personal and social habits
Prepares,
assists and instructs service users in food preparation, and cleaning of residences and vocational program areas
Supports service
users in identifying and acquiring valued social roles
Works with
each person receiving services to increase motivation; provides consistent reinforcement to learning, continuous assessment
of level of functioning, and continuous feedback to each individual for all learning activities
JOB REQUIREMENTS
Education - High
school diploma or Equivalent(GED) (Required)
Experience - Experience working with Personal Care individuals (Preferred)

We are currently looking for several full-time Qualified Professionals (QP) to work from our Brunswick County office
and we are also seeking a Qualified Professional (QP) for surrounding areas. Each QP will be coordinating services for clients
who have mental health/ developmental disabilities and/or behavioral issues. They will supervise Associate professionals and
be responsible for maintaining individual caseloads. Minimum of a bachelor’s degree in a related human service field
and two years of full-time postgraduate experience working with the population served are required. Individuals with a bachelor’s
degree in a non-related field must have a minimum of four years full-time postgraduate experience working with the population
served. Therapeutic Foster Care knowledge and experience is a plus.

Mental Health Qualified Professional Job Description: Direct care position that involves monitoring client care supervision, positive interaction
and role modeling, appropriate scoring of observed behaviors, and appropriate consequences or limit setting based on behaviors
and program definition. QP should maintain safety and security through close observation and monitoring of the clients. Promote
development of clients’ daily living skills through appropriate assignment, instruction and supervision of tasks. QP
should stimulate clients’ interest and cooperation in various skill building & symptom reduction activities, and
programs through positive interaction, instruction, supervision, feedback and encouragement. Completes accurate and timely
documentation on client's behavior as outlined by policy, reports observations to relevant staff and makes recommendations
based on observations.
Requirements
Education: Bachelor's degree is strongly preferred. There are Full time and per diem/visit ('as needed' or 'PRN')
positions available. Applicant must be able to work flexible hours. Must be rotate Crisis Line. Salary range: Depending on

Job # 4-2009.8: Registered Nurse Care Manager (RN)
Job description:
The RN is responsible for assessing and reassessing the nursing needs, including the physical, psychosocial, and environmental
needs of clients and their families. The RN develops an appropriate and individual care plan, collaborating with the client's
primary physician as needed, and is responsible for teaching/counseling clients regarding self-care and evaluating client
response to care provided. Acting as the clinical care coordinator for assigned individuals/ families, the RN initiates referrals
as necessary, supervises and directs Home Companions, Home Health Aides, families, and other caregivers regarding care of
the client.
Minimum qualifications:
Have current State Registered Nurse license and CPR certification. One year of continuous nursing experience or equivalent.
The ability to meet bonding criteria. Valid driver's license, available personal vehicle, and proof of current auto insurance.
Concise verbal, written, and organizational skills.
Desired qualifications:
At least one year of Home Health Care experience. Extensive assessment skills. Experience working with CARF standards.
At least two years of nursing experience, and ACLS certification.
Physical requirements:
Demonstrated ability to effectively communicate and interact with clients, families, co-workers, and all supervisors. Including
ability to proficiently follow and process written and verbal instructions in the English language. Hearing and visual capacities
sufficient for accurate communication with all patients and co-workers, including assessment and recording of clinical information.
The ability to lift, turn, and transfer clients in need.
